What is Toxic (Transient) Synovitis?

Toxic synovitis is a self-limiting inflammation of the hip joint synovium, most common in boys aged 3–10. It causes sudden limping, hip or thigh pain, and limited motion — often following a viral upper respiratory infection.

The critical step is distinguishing toxic synovitis from septic arthritis (joint infection), which requires urgent surgical drainage. Our team uses ultrasound, inflammatory markers, and Kocher criteria for safe rapid diagnosis.

Key Facts
  • Most common cause of acute hip pain in children — peak age 5–7 years
  • Usually resolves completely within 1–4 weeks with rest
  • Ultrasound shows hip effusion — aspiration differentiates from septic arthritis
  • Small risk of recurrence; 1–3% may develop Perthes disease
Treatment Options

Safe, Rapid Management

01

Ultrasound & Diagnostic Workup

Urgent hip ultrasound, CRP, ESR, and WBC to apply Kocher criteria and exclude septic arthritis.

02

Rest & NSAIDs

Bed rest and ibuprofen / naproxen provide rapid pain relief. Most children are walking normally within 1–2 weeks.

03

Aspiration (If Indicated)

Ultrasound-guided joint aspiration sent for culture when septic arthritis cannot be excluded — both diagnostic and therapeutic.

04

Follow-up & Perthes Monitoring

Pelvis X-ray at 6 weeks to exclude emerging Perthes disease. Recurrent episodes warrant closer monitoring.
