What is Spondylolysis?

Spondylolysis is a stress fracture of the pars interarticularis — a small bony bridge connecting the upper and lower facet joints. It occurs most commonly at L5 and is prevalent in young athletes in extension sports (gymnastics, football, weightlifting).

Most cases are managed conservatively with rest, bracing, and physiotherapy. If left untreated, bilateral spondylolysis can progress to spondylolisthesis (forward vertebral slip).

Key Facts
  • Affects up to 6% of the population; higher in athletes
  • Most common at L5 level; bilateral defects are more symptomatic
  • Acute lesions have high healing potential with bracing (3–6 months)
  • Chronic bilateral defects may require surgical pars repair or fusion
Treatment Options

Healing the Pars — Conservative to Surgical

01

Rest & Activity Modification

Ceasing provocative sports and relative rest for 6–12 weeks allows acute pars fractures to heal in most young patients.

02

Bracing

A rigid lumbosacral orthosis (LSO) unloads the pars and promotes healing in acute and subacute lesions.

03

Physiotherapy & Rehabilitation

Core strengthening and lumbar stabilisation are essential for return to sport and long-term recurrence prevention.

04

Direct Pars Repair

Buck's screw or pedicle screw-hook construct for bilateral chronic defects in young active patients — preserves motion segment.
