How limb lengthening works
Limb lengthening exploits the body's natural ability to regenerate bone. The surgeon makes a controlled cut in the bone (corticotomy), then gradually separates the two segments — 1 mm per day — allowing new bone to form in the gap (callus distraction). Over 3–4 months, 8–10 cm of new bone is created.
The choice between internal (PRECICE nail) and external (Ilizarov) systems depends on patient preference, anatomy, and the magnitude of lengthening required. The PRECICE system is fully internal and controlled by an external magnet — no external frame, better hygiene, and lower infection risk. The Ilizarov frame allows correction of angular deformities simultaneously with lengthening.

A titanium nail inserted inside the bone is lengthened by 1 mm/day using an external magnetic remote control (ERC). Fully internal — no pins through skin, no external frame. Superior patient comfort, lower infection risk, faster rehabilitation. Preferred for bilateral femur or tibia lengthening.
Ilizarov External Fixator
A circular frame attached to the bone via transfixion wires. Allows simultaneous angular and rotational deformity correction alongside lengthening. Preferred when bone deformity coexists with length discrepancy, or when PRECICE is anatomically unsuitable.
Physiotherapy Protocol
