What is Coccydynia?

Coccydynia is pain localised to the coccyx (tailbone), typically worsened by sitting, especially on hard surfaces, and by transitions from sitting to standing. It may follow trauma, childbirth, or arise spontaneously.

Most patients improve with conservative management. A small proportion require coccygeal nerve blocks or, in refractory cases, surgical coccygectomy — an effective procedure with high patient satisfaction.

Key Facts
  • More common in women (5:1 ratio) and overweight individuals
  • X-ray and MRI rule out fracture, tumour, or infection
  • Coccygeal manipulation under local anaesthetic can be highly effective
  • Coccygectomy has 80–90% success in carefully selected patients
Treatment Options

Targeted Treatment for Tailbone Pain

01

Conservative Care

Coccyx cushions, NSAIDs, and physiotherapy (including internal mobilisation by a trained therapist) resolve most cases.

02

Coccygeal Nerve Block

Image-guided injection of corticosteroid and local anaesthetic provides rapid relief — often repeated once or twice.

03

Ganglion Impar Block

Fluoroscopy-guided block of the ganglion impar — effective for refractory coccydynia and pelvic floor pain.

04

Coccygectomy

Surgical removal of the coccyx for refractory cases. Day-case procedure with 6–12 weeks recovery; 80–90% patient satisfaction.
